Player Application. This
application is for those of you who have previously gone on
an Ultimate Goal Trip. THIS IS THE ONLY FORM WE WILL NEED
FROM YOU. You don't need to have your pastor fill
out another recommendation.
Player

this is your first time to apply or go on a trip with UGM,
then you must fill out this form completely. You also need
to have the recommendation form completed by your pastor. We'd also like for your coach to fill out a recommendation for you as a player. Simply ask your pastor to visit
this page of our web site and fill out the online recommendations
for you. Once your application and pastor's recommendation form
are received, we will notify you of your acceptance.
Pastor's
Recommendation. This is
a recommendation for the young person from your church who
is applying to go on a trip next summer with UGM. Just click
on the link and fill out the form completely and honestly!
Your recommendation is kept in strict confidence.
